People say that Forgotten Lore is a must have but other than that I'm not sure what else to get.

As the Ancient Ones grow in power, investigators must work harder than ever to save our world. Personally I think the lower number of cards versus the other big boxes is a bit irrelevant.
It seems unfeasible to go through each deck and remove all the expansions except the one you're playing with. So if a theme for a particular expansion grabs your interest more, go for it. I haven't gotten up with the times of Eldritch Horrors and it seems that there's a lot of expansion for the game.
